Ah, these were good suggested modifications for any Yoga teachers. It is difficult to visualise, but now it is more clear. There are also ways to do fish on a low bench or firm matteress and the head hangs off onto another prop that can adjust in height for the student's body.
The book that Paulji mentioned is a good resource for any Yoga teacher who wants to teach Yoga as therapy. It may seem complex at first, but you will learn to help any student with many props and later you will become an inovator.
